**Job Summary:**

We are seeking a Certified Nursing Assistant (CNA) or Certified Home Health Aide (CHHA) to join our team on a PRN (as-needed) or part-time basis. The ideal candidate will provide high-quality, personalized care to residents, ensuring their comfort, safety, and well-being while supporting our healthcare team.

**Responsibilities:**
- Assist residents with activities of daily living (ADLs) such as bathing, dressing, grooming, and mobility.
- Monitor residents' health and report any changes or concerns to the nursing staff promptly.
- Help residents maintain a clean and comfortable living environment.
- Support residents in social and recreational activities to promote their emotional well-being.
- Adhere to safety protocols and infection control measures to ensure a safe environment for residents and colleagues.
- Document care and support provided to residents accurately and in a timely manner.
- Collaborate with healthcare team members to ensure continuity of care and support residents' individual care plans.

**Qualifications:**
- Current certification as a Nursing Assistant (CNA) or Home Health Aide (CHHA) required.
- Prior experience in an assisted living facility or similar healthcare setting is preferred.
- Strong communication and interpersonal skills to interact effectively with residents, family members, and team members.
- Ability to work independently and as part of a team in a fast-paced environment.
- Compassionate and patient-focused approach to caregiving.
- Flexible availability to work various shifts, including weekends and holidays, as needed.
