? Your Role as an LPN

As a Licensed Practical Nurse at Polaris, you’ll play a key role in providing exceptional resident care and supporting our nursing team. You will:

Administer medications and treatments
Assist in developing and implementing care plans
Supervise and coordinate CNAs in daily resident care
Communicate resident information accurately to nurses and physicians
Maintain compliance with State and Federal regulations
Ensure each resident receives compassionate, high-quality care
Perform additional duties as assigned to support the care team

✅ Qualifications

Current LPN license (multi-state or Delaware)
CPR certification required
Experience in long-term care preferred
Rehab, geriatric, or ventilator experience a plus
