Come join our in-house therapy team at the beautiful Highland Park Rehabilitation Skilled Nursing facility, in Aurora Colorado. Our 110-bed facility houses a resident demographic from varying acuity, giving you the opportunity to assist in the betterment of all walks of life amongst those we steward.
Work Schedule: Full-time and PRN
Pay Rate: $37.00 - $42.00 / Hour – depending on experience
JOB SUMMARY:
This position to ensure that quality care is delivered through monitoring of staff performance, physician contact, care planning and daily resident observation. This position also provides direct care to residents. All this is done according to nursing standards of practice, standard procedures, and established State and Federal regulations for skilled nursing facilities.
JOB DUTIES:

Assist routine daily direction and monitor the work of the nursing assistants on the unit, assuring a high standard of nursing care and satisfactory completion of tasks.

Follows company procedures for narcotic counting, pharmacy orders, and restraint reduction.

Communicate on a regular basis with the Nurse Manager or Director of Nursing regarding observations, and suggestions of problem areas, resident and employee activities, concerns, needs, conditions, etc.

Documents any change in patient's conditions including but not limited to nutritional problems, infections, skin conditions, weight loss or any other noticeable changes.

Participate in report at the beginning and end of shifts to assure proper information about residents is communicated.

Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.

Assist in the admission, orientation, and observation of new residents, assure that physician orders for diets, therapy, medication, etc. are transcribed and communicated according to procedure.
QUALIFICATIONS / EDUCATION / EXPERIENCE:

Currently licensed as an LPN in Colorado

Prior experience in a nursing community
-
The ability to read, write, speak, and understand the English language

The desire to care for residents and act with integrity

The ability to work flexible hours

Possess a valid BLS Certification
