Ability to work independently, be self-directed, and adapt to unpredictable circumstances. Strong interpersonal and team-based skills and ability to work with diverse populations. Ability to work with various computer software applications and technologies required. Advancing guideline-directed medical therapy (GDMT) and facilitating advance care planning for Medicare and MA patients. Collaborating with primary care clinicians in ambulatory settings to coordinate care. Performing virtual assessments, patient education, triage, and escalation of care needs. Assessing medically complex patients and acute exacerbations of chronic conditions. Coordinating care with a multidisciplinary team including specialists, social workers, physical therapists, and palliative care providers. Documenting all care provided and maintaining patient confidentiality. Practicing within the RN scope under the supervision of an advanced practitioner.
