Summary/Objective

The Admissions Licensed Vocational Nurse is responsible for evaluating and coordinating the flow process involved in taking calls, and facilitating the acceptance and triage of calls throughout the admissions and transfer call center. Serves as the primary liaison between physicians, hospitals and other care providers who seek to refer, schedule or transfer patients into the hospital. Provides clinical expertise, close medical communication and quality assessment for the successful referral, admission, and transfer of patients to the facility.

  1. Reviews documentation related to the patient’s condition and applies accepted criteria to determine if appropriate for admission and appropriate status for the patient upon admission per accepted criteria. Communicates appropriately with accepting/admitting physicians and hospital administration to ensure that patient safety and regulatory issues are address properly. Addresses and resolves issues arising from increased census and decreased bed availability. Communicates with the transferring facility to obtain accurate insurance information.
  2. Communicates with the surgery department, emergency department, nursing units, physician offices and outlying facilities to ensure appropriate placement of patients in a timely manner. Evaluates, processes, tracks, documents and coordinates patient transfers for the successful referral and transfer of patients to the facility.
  3. Ensures admissions, prioritizes/assigns beds based on appropriate level of care and patient aggregation. Collaborates with physicians regarding documentation required to accurately describe patient condition to determine to allow for appropriate diagnosis and level of care.
  4. Provides data for reports pertaining to the Admissions/Transfer Coordinator review function to provide basis for evaluation of accuracy in admission screens and for initiation of corrective action when necessary as well as reports for physician advisor services.
  5. Maintains appropriate communication channels with all levels of personnel for continuity of care. Notifies Directors of particular problems related to patients, families, personnel, or physicians. Notifies Administrator-on-call (or designee) of problems requiring administrative intervention.
  6. Demonstrates professional and technical knowledge related to utilization review, medical terminology, and appropriate levels of care, treatment modalities, statutes, and health care delivery systems. Understands, articulates, and ensures compliance with State Nurse Practice Act, Texas Department of Health regulations, regulatory agency standards and policies of the organization

Required Education and Experience

Graduate of an accredited nursing program

3 - 5 years related experience

Required License/Certifications

Current LVN license in the State of Texas
