Now Hiring: Licensed Practical Nurse (LPN)
Key Responsibilities of the LPN
As a vital member of our clinical team, the LPN is a bedside leader ensuring the highest standard of daily clinical care, accurate execution of physician orders, and precise documentation.
Medication Management: Prepare and administer all prescribed oral, injectable, and topical medications and treatments timely and accurately as ordered by the physician, strictly following all established safety protocols and "five rights" of medication administration.
Order Transcription & Communication: Accurately transcribe physicians’ orders to MARs (Medication Administration Records) and treatment sheets. Ensure prompt and effective notification of the pharmacy for all new orders, refills, and changes, following up to prevent interruptions in care.
Clinical Monitoring & Assessment: Monitor patient/resident health status, including taking and recording vital signs, performing routine assessments, accurately assessing wounds, and promptly reporting any significant changes in physical or mental condition immediately to the supervising Registered Nurse (RN) or Director of Nursing (DON).
Documentation Excellence: Complete accurate and thorough daily documentation in the electronic health record (EHR), including charting patient status, interventions performed, outcomes, and response to care, ensuring adherence to facility policy and legal standards.
Interdisciplinary Teamwork: Participate actively in shift reports and effectively communicate and coordinate care plans with CNAs, RNs, therapists, and other care team members to ensure seamless continuity of care and optimal resident outcomes.
Regulatory Compliance: Rigorously adhere to all federal, state, and local regulations (including HIPAA), as well as facility policies and procedures to ensure a safe and compliant environment.
Unit Support: Assist the supervising RN with admissions, discharges, and emergent situations, performing all other duties as assigned by the Director of Nursing or Unit Manager to maintain optimal unit operations.
Qualifications
License: Must possess a current, Active, and unrestricted Licensed Practical Nurse (LPN) License within the state of Wisconsin.
Skills: Demonstrated strong organizational skills, excellent attention to detail, and a genuinely compassionate approach to elderly and rehabilitative care.
Experience: Previous experience in a long-term care, skilled nursing, or sub-acute facility is highly preferred, but all dedicated LPNs are encouraged to apply.
Abilities: Must be able to manage time effectively, function independently, and maintain composure in a fast-paced clinical setting.
