Requirements:

Provides direct patient care in a home setting based on a specific care plan and physicians’ orders. This includes a general assessment, teaching, education, checking vitals, drawing labs, changing wound dressings and vacs, personal care, etc.
Timely and accurately reports resident/patient changes, needs, and/or requirements to the Registered Nurse and/or other managing staff and physicians.
Collaborates with patients’ clinical team with concerns, goal setting, and changes to care plan.
Accepts physicians’ orders.
Participates in Individual patient case conference team meetings.
Accurately and timely completes required documentation in EMR of nursing care to meet all state and Village requirements.
May delegate appropriate tasks to patient caregivers and follows up on assignments.
Adheres to all safety regulations and requirements.
Other responsibilities may be assigned by the supervisor as warranted by business needs.
Must be a Licensed Practical Nurse by the Missouri State Board of Nursing.
Current Basic Life Support (BLS) certification or ability to obtain before direct patient contact.

Preferred Qualifications Role Purpose The Staff LPN Ambulatory is responsible for providing direct patient care under the direction of a physician or other licensed provider in an ambulatory care setting. Assigned tasks may include rooming patients, obtaining vitals, administering medications in-office, performing point of care lab testing, assisting with procedures and triaging patient phone calls. All responsibilities are to be carried out in accordance with applicable state regulations. Follows HIPAA, CLIA, OSHA, Medication Management and Infection Prevention procedures for cleaning and sterilization of equipment and/or instruments. Performs clinical skills, such as phlebotomy, EKGs, point of care testing and medication administration. Assists with medical office procedures. Obtains vital signs, patient history and maintains accurate and prompt charting in the patient's record.
