Overview

The Ambulatory Care Nurse (ACN) is a Registered Nurse responsible for providing nursing care management services to the assigned populations. The ACN utilizes a collaborative approach to anticipate, integrate, and coordinate the patient?s plan of care and maximize the ability to remain in a community setting. In collaboration with the physician, the nurse identifies at risk patient populations who require care management services. The ACN serves as a patient/family advocate and acts as a communication link between the physician, patient, family, and interdisciplinary health care team, as well as community resources. The Ambulatory Care Nurse understands the needs of the organization and supports the mission, values, and management of patient care services. The ACN actively supports and incorporates ANA Scope and Standards of Nursing Practice, North Carolina Board of Nursing Practice Act, WakeMed's Division of Nursing Professional Practice model, and WakeMed's Patient & Family Centered Care (PFCC) model.

Collaborates with interdisciplinary team such as interpreters, clerical team, social work, Duke Well, and other members of the patients care team Participates in quality/performance improvement programs and projects, product evaluation, outcomes evaluation studies and/or clinical research. Support the development of other staff and formal learners. Participate in the identification of clinical, operational and safety related performance improvement opportunities. Participate in own professional development by maintaining required competencies, identifying learning needs and seeking appropriate assistance or educational offerings. Perform other related duties incidental to the work described herein. Job Eligibility Requirements: Work requires graduation from an accredited Associate's Degree in Nursing or Nursing Diploma program. All registered nurses without a Bachelor's degree in Nursing (or higher) are encouraged to enroll in an appropriate BSN program within two years of their start date but must complete the BSN program within seven years of their start date. Exception: Registered nurses hired between July 1, 2014 and April 11, 2021 without a Bachelor's degree in Nursing (or higher) are encouraged to enroll in an appropriate BSN program within two years of their start date but must complete the program within seven years of their start date. Registered nurses hired before July 1, 2014 are not required to enroll in a BSN program to remain in this job classification. Must have current or compact RN licensure in the state of North Carolina. BLS certification required. Job Hours: The clinic is open Monday-Friday 8:00 am-5:00 pm and Saturday from 8:00-12pm. Nurses are scheduled to work 1 Saturday every 6-8 weeks.
