This position will be 2 days working remote processing data that come in from patients' pacemakers and defibrillators and 2 days answering questions/taking calls from patients regarding Electrophysiology inquires. Must have experience with Electrophysiology, in depth cardiac rhythm understanding and identification. Must be able to work independently and with a team. Must reside in Indiana as there may be times when you may need to come into the clinic for mandatory training. Orientation will be in person for several weeks and then individual will be allowed to resume remote roles. Schedule: Mon, Tues, Thurs, Friday - 7:00 am - 5:30 pm No weekends! No holidays! Requires an Associates of Nursing (ASN); Bachelors of Nursing (BSN) preferred. Requires an active Registered Nurse (RN) license in the state of Indiana or an active Nurse Licensure Compact (NLC) RN license. Requires that the RN has graduated from a nationally accredited nursing program. Requires Basic Life Support certification through the AHA. Other advanced Life Support certifications may be required per unit/department specialty according to patient care policies. May require ability to assess patients without face-to-face interaction. Requires awareness and understanding of legal implications of patient care activities and performance as a Registered Nurse.

RN Case Manager - Primary Care

Overview The Case Manager plans and coordinates care for patients from pre-admission through post-discharge by working collaboratively with the multidisciplinary team. Ensures that patients are assigned the appropriate level of care, receive the necessary services, and transition smoothly to the appropriate post-discharge setting. Performs accurate, timely, and effective discharge planning. Works with third-party payers to ensure coverage and administer education and prevention programs. ________________________________________ Position Overview: The RN-Case Manager for Primary Care plays a critical role in ensuring seamless patient transitions across care settings. This position involves assessment, care coordination, and communication with patients, families, providers, and payers. The nurse assists with a smooth transition from hospital discharge to care at home and facilitates patient education and safety. Position Schedule: Full-time, day shift, Monday-Friday - Remote ________________________________________ Key Responsibilities: • Collaborate local Primary Care offices to assist with patient appointment scheduling. • Collaborate with the multidisciplinary team to develop comprehensive care plans. • Work with third-party payers to verify coverage and facilitate authorization for services. • Educate patients and families on post-discharge care, safety, and prevention strategies. • Assist with third-party denials, appeals, and understanding contractual arrangements. • Ensure compliance with Medicare, Medicaid, insurance, and regulatory guidelines. • Utilize various software applications including Windows, Cerner, MCCM, and SMS for documentation and communication. ________________________________________ Qualifications & Requirements: • Education: - Associate's Degree in Nursing required. - RNs hired after January 1, 2013, must complete their BSN within 5 years of hire. • Licensure: - Active RN license in Indiana or Nurse Licensure Compact (NLC). • Certifications: - Case Management certification preferred. • Experience: - 3-5 years of clinical experience in acute care required. • Knowledge & Skills: - Knowledge of InterQual Acute Level of Care Criteria and federal coverage guidelines is preferred. - Familiarity with Medicare/Medicaid, insurance, and regulatory guidelines is preferred. - Ability to understand third-party contractual arrangements and denial/appeal processes is advantageous. - Proficiency with Windows, Cerner, MCCM, SMS, and other relevant software applications. - Strong clinical background with excellent assessment and communication skills. ________________________________________ Core Competencies: - Connect to Promise: Demonstrates commitment to IU Health’s mission, vision, and values by exhibiting behaviors and delivering results aligned with the organization’s strategic goals. Overview Location: Bloomington Hospital Position: Clinical Nurse-Ambulatory Quality Coordinator Shift: 1.0 FTE Day Shift 8a-5p The Clinical Nurse-Ambulatory Quality Coordinator develops, coordinates, implements, and evaluates ongoing quality improvement activities for assigned populations. They work independently, lead specific work groups, and perform thorough analysis of gaps in clinical care and barriers to care, aligning interventions with the organization’s strategic plan. This role facilitates the organization’s performance improvement processes in collaboration with project team leaders and provides training and education to staff and physicians in performance improvement methodologies, data analysis, work simplification techniques, and related subjects. ________________________________________ Position Overview: The Clinical Nurse-Ambulatory Quality Coordinator is a member of the IU Health Medical Group’s statewide quality department. Position supports IU Health Medical Group Specialty Care practices with quality initiatives, metrics support, and data reporting. Position requires the ability to train providers and clinical support team members on quality tools, quality metric specifications, and workflows to support closing gaps in care.
