Job Description

This position is accountable for oversight of patient care delivery whenever the Nursing Directors are not in the hospital. This RN makes rounds to the nursing units, collaborates with staff to solve clinical and staffing issues, addresses and documents practice variations and staff non-compliance to HR policies. Nursing metrics are assessed and staff coached/mentored to improve outcomes when deficits are identified. This RN also serves as a patient advocate, communicates with support departments as appropriate to resolve issues. Following the chain of command, communicates appropriate issues to AOC, Nursing Leadership, and Ancillary Departments.

Education
Bachelors Nursing Required or BSN completed within 5 years of hire.
Graduate of an accredited nursing program required.

Experience
5 Years Acute care, critical care, ED nursing experience preferred; with
2 Years Demonstrated in leadership responsibilities preferred.

RN Nursing License required *
BLS Certification (Basic Life Support) - American Heart ‘Healthcare Provider’ (HCP) - AHA approved required *

Identify patients who have had a change in condition and escalate care to provider or EMS services as appropriate. 8. Performs face-to-face patient visits in order to update medical/surgical/family history, review current medications and allergies, assess social determinants of health, provide appropriate health screenings, assess for functionality, and review medical record for gaps in care. Conduct shared decision-making conversations with patient in order to close care gaps. Report findings to provider. 9. Assists patients in developing advanced care planning. 10. Serve as liaison to providers, patients and families for coordination of services. 11. Maintains EMR databases on care managed population. Maintain accurate and timely documentation and billing.
