About the Role

We’re seeking a compassionate Licensed Practical Nurse (LPN) to join our hospice team—available full-time, part-time, or PRN. As an LPN, you’ll provide direct patient care under the supervision of a Registered Nurse, following individualized care plans and ensuring every patient receives the highest standard of comfort and support. You’ll collaborate closely with an interdisciplinary team to deliver care that meets physical, emotional, and spiritual needs.

  • Provide direct patient care as outlined in the plan of care established by the Registered Nurse.

  • Implement nursing interventions based on assessments and care plans.

  • Document care accurately and timely in the EMR—right at the bedside.

  • Educate and support patients, families, and caregivers to promote comfort and understanding.

  • Manage equipment and medical supplies efficiently.

  • Provide appropriate pain and symptom management, and communicate changes to the RN or provider.

What You’ll Bring

  • Graduate of an accredited practical or vocational nursing program.

  • Current LPN license in Virginia.

  • CPR certification and at least two years of nursing experience (community health, hospice, or medical-surgical preferred).
