A Brief Overview
The Patient Care Manager is a Registered Nurse who provides unit leadership and works in partnership with nursing, the healthcare team, hospital/system leaders to develop a quality practice setting that support professional nursing and quality patient care. The Nurse Manager is accountable for the effective delivery of competent, compassionate, and efficient patient care. Responsibilities include coordination of patient care; supervision of a professional team that includes multiple nursing and other staff as appropriate; administrative and budget management; troubleshooting of problem areas; organizational change management; ensuring compliance with accreditation and regulatory standards; quality assurance/quality improvement; safety and risk management requirements as well as actualization of hospital and unit policies, goals and objectives. Further, improving unit operations, enhancing communication and feedback, customizing education and staff roles, and accepting organizational accountability for services provided to recipients are critical outcomes expected of this role. The Nurse Manager role is critical to hospital operations and minimally accomplishes the following essential duties. Patient Care Manager I vs. II designation is based on span of control, hours of operation, and level of unit complexity.

What you will do
  • Clinical Practice: Ensures the effective delivery of competent, compassionate, and efficient patient care by planning, organizing, and directing the nursing services of unit(s), including the assessment of patients, implementation of physicians' orders, treatment plans and medical procedures, administration of medication and record keeping of same as well as by monitoring and evaluating patient care processes and outcomes for the patient care unit(s); coordinates nursing activities with those of other services or disciplines. Demonstrates clinical competency in assigned specialty area role models nursing practice consistent with the organizational and department mission, vision and values.
  • Communication: Communicates in a clear and effective manner to enhance team effectiveness, manage conflicts, engage employees and interdisciplinary colleagues in unit activities and performance changes. Collaborates with others within the unit and with all hospital departments and disciplines to enhance problem solving, decision making and performance improvement.
  • Fiscal Accountability: Plans and forecasts the department budget by calculating required FTES, planning annual budgets, interpreting payroll and other key fiscal reports, projecting workload as well as monitoring and enhancing productivity. Performs variance analysis by interpreting financial performance and identifying key drivers. Identifies unit/capital budget needs in collaboration with physician and nursing colleagues. Plans and implements staff scheduling, position controls and attendance control procedures.
  • Human Resources: Directs the personnel management of the employees assigned to the department consistent with state and federal labor laws, collective bargaining agreements, and nursing service and hospital philosophy, goals and policy. Ensures that new staff is interviewed, hired, trained, oriented, precepted and evaluated. Prepares and conducts employee performance appraisals. Approves transfers, promotions and other decisions affecting employee pay and status. Participates in initiating and documenting disciplinary actions and responding to grievances.
  • Leadership: Assumes twenty four (24) hour responsibility and accountability for the operation of the assigned patient care unit(s), service(s), staff, and facilities. Plans, organizes and directs short and long term departmental activities in accordance with hospital goals and accrediting organization standards. Provides leadership and direction for the staff in the patient care unit(s) while supporting and actualizing the goals of the department, division, service, and hospital. Functions as a resource to staff and interdisciplinary team members and interdepartmentally, consults on nursing care and professional nursing practice.
  • Performance Improvement: Assumes responsibility for accessing and utilizing appropriate date in order to monitor unit operations. Accountable for achieving patient and staff satisfaction targets for the organization and unit of responsibility. Develops, implements and evaluates process improvement projects/process in order to continually monitor and improve patient care.
  • Professional Development: Facilitates professional development and clinical growth of nursing staff through implementation and maintenance of unit specific competencies. Promotes and facilitates educational opportunities based on assessment of staff needs.

Education Qualifications
  • Bachelors and/or Masters in Nursing from an accredited college or university.

Experience Qualifications
  • Three (3) years of progressively responsible and directly related work experience.

Base Pay Scale: Generally starting at $89.01 - $117.94 per hour

The salary of the finalist selected for this role will be set based on a variety of factors, including but not limited to, internal equity, experience, education, specialty and training. This pay scale is not a promise of a particular wage.

Overview JOB TITLE: Staff Nurse II CC/Department: 7010/Emergency Services Reports to: Administrative Director of Emergency Services JOB SUMMARY: The Staff Nurse is a professional caregiver who assumes responsibility and accountability for the assessment, nursing diagnosis, planning, implementation and evaluation of care for patients and their families in compliance with the standards for care at Seton Medical Center and Nurse Practice Act. Responsibilities Clinical Practice: Nursing 1 Assesses, collects and documents health assessment data focusing on the physical, emotional, spiritual, psychosocial and educational needs of the patient. a) Interviews the patient/family/significant other to complete Triage information. b) Identifies actual and potential needs and determines priority. c) Identifies barriers to learning. After initial triage evaluation, completes and documents an assessment, focusing on abnormal systems and actual and potential problems. Performs ongoing assessment as indicated. 2 Plans nursing care, based on priority of physical and psychosocial-emotional needs. a) Develops realistic short-term goals for each problem, involving the patient, family or significant others. b) Plan demonstrates understanding of age-related needs of the patient. c) Negotiates with patient, family, significant other and health care team members in planning for education and discharge needs. 3 Implements the plan of care including prescribed procedures and treatments. Organizes and prioritizes care based on individual needs and unit activity. Assesses for changes in patient’s condition and appropriateness of the plan. Initiates interventions using appropriate clinical expertise and knowledge. As Primary Nurse, coordinates the activities of health care team members in completing diagnostic and therapeutic interventions. 4 Evaluates the effectiveness of nursing care delivered to reach the expected outcome. a) Evaluates the plan of care according to the current condition and changing needs of the patient. b) Accurately and concisely documents the patient’s status and response to the care delivered. Clinical Practice: Emergency Nursing 6 Utilizing professional nursing knowledge and skills. Appropriately assesses for: Physical and psychosocial-emotional needs. Barriers to learning. Educational/learning needs. Discharge needs. Age-related needs. Issues related to assault or abuse. 7 Analyzes patient related data, such as vital signs, past medical history, physical exam findings, laboratory values and other information, including age-specific criteria, to determine appropriate care. 8 Utilizes advanced, specialized knowledge and skills to implement interventions for all Emergency Services patients, by prioritizing needs, and using a patient centered approach. 9 Provides health education and teaching to patient’s, family members and/or significant others, that is age specific, providing for any barriers to learning. Clearly communicates health, treatment and discharge information, ensuring the patient, family member or significant other understands all discharge and follow-up instructions. 10 Effectively performs procedures, such as initiation of IV catheters and fluids, medication administration, dressings, patient teaching, etc., within their scope of practice, and unit, department and Medical Center policies and procedures. 11 Effectively performs other nursing procedures/skills, such as cardiac monitoring, rhythm interpretation, defibrillation, monitoring of conscious sedation, ABGs etc., based completed specialty training and their scope of practice, and unit, department and Medical Center policies and procedures. 12 Appropriately recognizes and quickly responds to emergency medical situations. a) Accurately assesses “A-B-Cs.” b) Initiates appropriate emergency treatment, i.e., CPR, O2, etc. c) Communicates critical information to other team members. 13 Maintains all required credentials and training (CA RN license, BLS, ACLS, PALS, NIHSS, AB508). 14 Remains current on emergency treatments/practices/procedures through attendance at staff meetings, continuing education events, in-services and the review of professional literature. 15 Integrates/applies new knowledge from education and research into clinical practice. 16 Demonstrates knowledge of and competence in age-related patient care. 17 Consistently demonstrates knowledge of state and federal regulations, such as Title XXII and COBRA/EMTALA. Follow all TJC, CDPH, CMS guidelines. 18 Advocates for public health and safety through patient and family education. 19 Consistently completes required documentation of the Emergency Services visit. 20 Consistently demonstrates caring and compassion through behaviors and interactions in performance of all duties, recognizing the inherent worth and dignity of patients, families, other staff and physicians. 21 Will perform duties of Relief Charge RN if called upon to do so. 22 Adheres to departmental and medical center policies on scheduling, overtime, sick time and time off. 23 Arrives punctually, takes appropriate time for breaks, use sick time appropriately. Qualifications QUALIFICATIONS/JOB REQUIREMENTS: EDUCATION: Graduation from an accredited school of professional nursing. Baccalaureate degree in Nursing preferred. EXPERIENCE: Staff Nurse II – Minimum of six months Registered Nurse experience. Six months current ER & triage experience or completion of Seton ER Nurse Training Program. CERTIFICATIONS/LICENSURE: Active California Registered Nurse licensure Basic Life Support certification (BLS) required ACLS certification required PALS certification required NIHSS certification required Workplace Violence Prevention De-escalation 8-hr (AB508) certification or CPI certification required Preferred: NRP certification. OTHER SKILLS, ABILITIES & KNOWLEDGE: Diffusing Assaultive Behavior Training Knowledgeable of the California Nursing Practice Act. Able to understand and follow all policies, procedures, guidelines, rules and regulations of Nursing Service, Seton Medical Center, Title XXII. Excellent communication and customer service skills. Proficient in Evident computerized documentation system preferred.
