Position Summary:

Pick your own shifts at various locations based on the needs of Monarch Healthcare Management! This is a full-time or on-call position with weekend requirements.

The LPN participates in the development and modification of the resident Plan of Care, implementing interventions that are delegated, ordered, or prescribed by the RN or physician. As an LPN you are providing residents with the highest quality of direct routine care, under the supervision of the RN and in accordance with the Minnesota BON standards, the Resident’s Bill of Rights and the facility Quality Assurance and Performance Improvement (QAPI) program.

The Monarch Float Pool was created to help offer staffing assistance to our own Monarch facilities. Pick your own shifts based on the needs of Monarch Healthcare Management. Travel is expected for this position and there are benefits available. Qualifications:

  • Completion of a professional nursing education program approved by the Minnesota Board of Nursing.

  • Current Minnesota License as a Practical Nurse.

  • Current Certification in Cardiopulmonary Resuscitation (CPR) and emergency care or be willing to get one upon hire.

  • Experience in skilled nursing and/or assisted living facilities.

Keeps patient records and reports according to policy. Provides leadership and directs functions of Certified Nursing Assistants. Assists with admissions, discharges, transfers and appointment scheduling. Notifies charge nurse of any change in patient condition. Investigates and reports any allegations of abuse or caregiver misconduct and ensures residents are treated with respect and dignity. Administers medications and treatments as prescribed by physician and psychiatrist. Contacts medical provider and family of residents related to change of condition or concerns. Receives MD orders, makes rounds with MD's, transcribes orders, and communicates orders to nursing staff and pharmacy. Assists in care and monitoring of IV's per facility policy. Monitors and assists residents with psycho-social events. Provides support, information or education to residents or family members as needed. Other duties as assigned.
