Cedarhurst of Topeka, a regional healthcare provider and true friend of older adults, seeks a service-minded, career-oriented individual to join our team as a Licensed Nurse LPN  at our Cedarhurst of Topeka community. 

As a Licensed Nurse, you will have the opportunity to connect with, and positively impact the lives of, our residents daily! You will be responsible for the planning, implementation, and evaluation of all nursing care prescribed and provided to the resident.

As a Licensed Nurse, you will impact the lives of our residents by

  • Providing services in accordance with nurse practice act and state/federal nursing center regulations.
  • Working with the Director of Nursing to provide direction and oversight to the care staff regarding the resident’s needs.
  • Reviewing and revising assessments and care plans at admission, periodically per regulation, and at change in resident physical or psychosocial condition.
  • Reviewing and transcribing physicians orders and providing medical treatments as ordered by physician along with assisting physician with resident examination, as needed.
  • Initiating a resident education plan according to the individualized needs of the resident, as prescribed by Physician and/or community policy including resident and family instruction.
  • Observing residents and communicating condition changes to physicians, health care proxies, and supervisors and provides timely interventions as instructed.
  • Administering and documenting medications in accordance with the five rights: right medication, right route, right resident, right time, and right dose.
  • Supervising and monitoring medications for residents who self-administer if applicable.
  • Documenting all pertinent/relevant observations, red flags, incidents, interventions, communication, clinical procedures, and daily medication assistance/care throughout the shift as indicated.
  • Assisting and monitoring residents with meeting their nutritional/hydration needs and documents on the nursing record appropriately.
  • Assisting residents with activities of daily living as needed, including bathing/showering to schedule, oral care, toileting, dressing, etc.
  • Assisting residents in scheduling medical, dental and laboratory appointments.
  • Utilizing proactive intervention use, monitoring, and safety in the rooms and rounds.
  • Answering residents call light promptly.
  • Assisting residents with transfers and ambulation as directed, using proper body mechanics.
  • Taking temperature, pulse, blood pressure, and other vital signs to detect deviations from normal and assess the condition of resident as requested.
  • Maintaining a restraint free environment at all times.
  • Supporting fall prevention programs for all residents in need.
  • Responding to life saving situations based upon nursing standards, policies, procedures, and protocol.
  • Responding as appropriate to personal calls from other health center units in the community.
    • Duties in this setting may include but are not limited to providing First Aid, calling emergency personnel (dial 911), and/or calling family members at request of residents.
  • Reviewing resident service plans for assisted living and memory care.
  • Encouraging residents to participate in scheduled facility activities providing assistance in getting to activities as needed.
  • Assisting in post-mortem care as instructed.
  • Participating in oversight and management of medication delivery system to include timely order entry, receipt and tracking as well as daily MAR documentation, narcotic count, and checking in medications in timely manner as assigned to ensure resident compliance to medication regime.
    • Notifying pharmacy and supervisor of issues and seeking to resolve them in a timely manner.
  • Promoting the physical, personal, and emotional well-being of each resident and striving to maintain the ultimate goal, which is to return the resident to their maximum level of self-care and independence, when possible.
  • Understanding the residents’ Advance Directives and following their wishes.
  • Ensuring effective communication is being utilized by community staff with the residents’ families concerning residents’ care, encompassing their physical, personal, and emotional wellbeing.
  • Working well with others and taking direction from management, as well as taking initiative with the capability to engage in conflict intervention and resolution for residents, families, employees, and others as needed.
  • The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is occasionally required to stand/move, communicate, and identify written information.The employee is occasionally required to climb or balance and stoop, kneel, crouch, or crawl and lift and/or move up to 50 pounds independently; occasionally lift and/or move up to 200 pounds with assistance.
  • Other duties as assigned.

To become a Licensed Nurse, you will need –

  • Graduation from an accredited nursing school and a current state license for LPN or RN is required at all times.
  • 1+ years of nursing experience preferred but new graduates are welcome.
  • CPR and BLS certification preferred.
  • Be able to demonstrate proficiency in Internet browsers (e.g., Explorer, Chrome, and Fire Fox), Microsoft Office programs including Outlook, Excel, Word and Publisher, electronic health records (EHR) systems, and any HR related systems if applicable.
