LPN Long Term CareJob Description
Renders general care procedures on residents/patients, assessment of resident/patient, documents these procedures, educates other caregivers, communicates specific job functions with other employees and assists other nursing duties which aides on the overall delivery of care.
6:00pm - 6:00am with weekend and holiday rotation
Pay based on experience starting at $22.72
Responsibilities
Adequately assesses and reassesses pain. Utilizes appropriate pain management techniques. Educates the resident and family regarding pain management., Administers medications and treatments according to protocol. Clarifies and reviews physician’s orders and takes appropriate action., Assesses residents on admission and at least quarterly, as well as when conditions change. Documents appropriately including Minimum Data Sets., Coordinates discharge planning with all team members., Documents completely and thoroughly. Assures that documentation reflects changes in patient condition and notification to appropriate individual has occurred., Enhances patient care and efficiency of unit by optimizing workflow of all team members. Delegates to other members and holds them accountable for delivery of quality patient care. Provides education and constructive feedback to team members., Notifies physician of changes in resident’s condition and follows through until appropriate action is taken. Documents changes in resident's condition. Orders ancilliary testing and prescriptions., Performs general patient care and Activities of Daily Living (ADL) procedures., Plans and evaluates care in collaboration with appropriate disciplines for assigned patients. Leads and participates in daily care to modify plan of care. Identifies patient problems on integrated flowsheets and documents against the plan of care., Responds to patient's call light and other indicators for assistance promptly and courteously and follows through to meet patient's needs. , Teaches patients and family members about the plan of care and documents education. Keeps both patient and significant others informed.Requirements
