SCHEDULE: Full Time, 12 HR Night Shift,

POSITION SUMMARY

Assists the RN in gathering assessment data. Performs and assists with procedures. Documents care provided. Operates clinical equipment and patient information systems. Maintains an atmosphere which ensures South Georgia Medical Center’s mission, vision, values, code of conduct, goals, policies and standards are consistently supported and enforced.

KNOWLEDGE, SKILLS & ABILITIES

Current LPN License in State of Georgia.
Current Healthcare Provider CPR required.
ACLS and PALS preferred within twelve months of hire
CPI required within eighteen months of hire.
One (1) year experience in related acute care specialty preferred.
Ability to work well individually and as a team member.
Ability to establish and maintain effective working relationships with patient/family units.
Ability to read and interpret policies and standards. et policies and standards.
Strong clinical skills in assessment, intervention, documentation and evaluation.
Ability to communicate clearly with, and care for diverse population of patients/family units of all ages, a variety of cultures, and/or persons occasionally exhibiting stressed behaviors.
Ability to operate clinical equipment, computer and telecommunications devices.

WORKING CONDITIONS - ADA INFORMATION

Works primarily in hospital setting. Requires considerable standing, mental effort, stooping, bending, lifting (in excess of fifty (50) lbs.), pushing objects (carts, beds, and wheelchairs). Likely to be exposed to blood and body fluids, including infectious diseases. Uses medical, equipment, telecommunications devices and patient information systems on a regular basis. Physical functional capacity for healthcare as defined by the Saunders Method (very heavy).

JOB PURPOSE OR MISSION: The Licensed Practical Nurse’s (LPN) primary role is to provide nursing care for patients who are experiencing common, well-defined health problems. The LPN functions as a member of the multidisciplinary health care team, and utilizes the nursing process in implementation of patient’s care. Those services are performed under the supervision of a registered nurse and utilize standardized procedures with predictable outcomes. The LPN adheres to standards and regulations as set forth by the facility’s policies and procedures as well as external credentialing bodies such as JCAHO, OMH and OSHA. ORGANIZATIONAL PERFORMANCE CRITERIA: Demonstrates commitment and dedication in meeting the overall corporate Values, Mission and Vision for the future. DESIRABLE MINIMUM QUALIFICATIONS: The LPN must be able to communicate effectively both orally and in writing, be highly organized, have an understanding and sensitivity to serve in culturally diverse and special needs population of the chronically mentally ill, geriatric, CD, adult psychiatric population as well as has basic knowledge of normal physical and emotional development. SPECIAL SKILLS AND KNOWLEDGE REQUIREMENTS: Current State of Georgia licensure as a Licensed Practical Nurse. Current certification in American Heart Association CPR or recertification within 6 months of hire date. Current certification in Crisis Prevention Institute (CPI) or obtain certification within 6 months of employment. Demonstrated proficiency in Age and Disability specific competencies demonstrated by successful completion of facility testing. Ability to speak English and communicate effectively, verbally and in writing. Must have basic knowledge of psychiatric nursing as required by entry level practices. PHYSICAL REQUIREMENTS: Must be able to stand on feet for long intervals and have dexterity to participate in physical restraint of patients when necessary. A certain amount of walking and standing are often necessary to carry out job duties. Lifting/carrying will routinely exceed 10 pounds and involves assisting patients in ambulation, positioning and ADL’s. May be required to lift, roll, or otherwise assist in moving patients.
