About the Role
We are seeking a compassionate Certified Nursing Assistant (CNA) to join our PACE Home Care team. In this role, you will provide essential care and assistance to participants in their homes, helping them maintain independence, comfort, and dignity. You will be an integral part of our care team, ensuring participants’ daily needs are met while fostering trust, safety, and respect.
What You'll Do
Assist participants with activities of daily living (bathing, dressing, grooming, oral care, eating, toileting, hand and foot nail care).
Support mealtime needs, including feeding assistance when necessary.
Help with laundry and light household tasks to maintain a clean, safe environment.
Safely assist participants entering and exiting program vehicles.
Monitor and promptly report changes in participants’ conditions to nursing staff.
Contribute to a positive, supportive home environment for each participant.
Perform other care-related duties as needed.
What We're Looking For
· Education: High school diploma or equivalent.
· Certification: Active, unencumbered CNA license in good standing and current CPR certification (required).
· Experience: Minimum one year working with older adults or individuals with complex care needs.
· Valid driver’s license, proof of insurance, and reliable transportation.
· Medical clearance for communicable diseases and up-to-date immunizations prior to direct participant contact.
