We seek a creative, enthusiastic, and passionate Registered Nurse (RN) Coordinator who is responsible for overseeing the delivery of nursing care to clients in their homes, ensuring compliance with agency policies, state regulations, and individualized care plans.

The position involves coordinating care, conducting assessments, supervising clinical staff, and providing direct patient care as needed. The position offers flexible scheduling to accommodate the agency's needs and the candidate's availability.

ESSENTIAL FUNCTIONS

Care Coordination:

  • Develop, implement, and evaluate individualized care plans in collaboration with physicians, clients, and families.
  • Coordinate with interdisciplinary teams, including home health aides, therapists, and social workers, to ensure comprehensive care delivery.

Clinical Assessments:

  • Perform initial and ongoing assessments of clients’ health status to determine care needs.
  • Monitor and document client progress, updating care plans as necessary.
  • Identify and address potential health risks or changes in condition, communicating promptly with physicians and families.

Staff Supervision and Training:

  • Provide clinical guidance and support to home health aides and licensed practical nurses.
  • Conduct in-service training and orientation for new staff to ensure compliance with agency standards.
  • Evaluate staff performance and provide feedback to promote quality care.

Compliance and Documentation:

  • Maintain accurate and timely documentation by agency policies and state/federal regulations.
  • Ensure compliance with infection control protocols, HIPAA, and other regulatory standards.
  • Participate in quality assurance audits and contribute to continuous improvement initiatives.

Communication and Collaboration:

  • Serve as a liaison between clients, families, physicians, and agency leadership.
  • Attend case conferences and team meetings to discuss client progress and care strategies.
  • Respond to client and staff inquiries in a professional and timely manner.

Other essential functions

  • Ensures compliance with company policy and all federal, state, and local regulatory bodies and conditions of participation
  • Remains informed and updated on home health regulations and standards
  • Perform other such tasks and assignments as directed by the leadership of Right at Home

EDUCATION / SKILLS / ABILITIES / AVAILABILITY

  • Registered Nurse (RN)
  • Working knowledge of HIPAA, The Joint Commission, and Medicare guidelines, and any related federal and state rules and regulations
  • Ability to effectively communicate with good oral and written communication skills
  • One year of supervisory experience required
  • Home care experience required
  • Read, write, speak, and understand English as needed for completion of the job
  • Current CPR certification
  • Have a valid driver’s license and use of an insured automobile or access to adequate transportation.
  • Must be able to demonstrate a two-person transfer with a Hoyer Lift for educational purposes
