CNA Home Health Part-time

Encore Village of Schaumburg is currently hiring for a part time C.N.A. in our Home Care Department. The Home Care C.N.A. provides personal care and supportive services that are intended to assist clients in accordance with individualized service and nursing treatment plans, which promotes their physical and emotional wellbeing and encouranges them to reach their highest level of independence.

Responsibilities:

Essential Functions: 

  • Provides personal care to Home Care Residents.
  • Recognizes an emergency situation and acts accordingly.
  • Completes necessary documentation associated with visits.
  • Maintains current knowledge in area of specialty and activities within agency.
  • Performs other duties as assigned.
  • Assists in teaching healthcare procedures to residents and/or caretaker including safety measures.
  • Documents each home visit including care rendered in EHR.
  • Submits documentation to office according to established time frames.
  • Participates in orientation of new home care staff as to the role and duties of Home Care Aide. Provides in-services, as appropriate.
  • Reports any changes or emergencies to supervisor immediately.
  • Maintains confidentiality of all residents.
  • Utilizes universal precautions at all times.
  • Updates resident service plan and assignment as changes arise.
  • Attends scheduled staff meetings and in-services as appropriate.
  • Provides feedback to supervior regarding resident's ability to care for needs in apartment.
  • Work as a team member to assure residents receive timely, competent, compassionate and respectful care.
  • Answers and directs phone calls to the agency appropriately.
  • Completes skills tests annually.

Education:

  • Successful completion of basic course of instruction for C.N.A. which has been approved by Illinois Department of Public Health
  • Current CPR Card
