RN Home Health Full-time

Overview
We are seeking a dedicated and compassionate Registered Nurse (RN) to join our Home Health team. The ideal candidate will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being. This role requires a strong understanding of various medical practices, including pain management, discharge planning, and community care. The RN will work closely with patients and their families to develop personalized care plans that promote recovery and enhance quality of life.

***We are currently looking for Private Duty hours for 8 or 12 hour shifts, one-on-one focused patient care!***

Responsibilities

  • Conduct comprehensive assessments of patients' health status and needs in their home environment.

  • Develop and implement individualized care plans based on patient assessments and medical history.

  • Administer medications and treatments as prescribed.

  • Monitor patients for changes in condition, providing timely interventions as needed.

  • Educate patients and families on health management, including pain management strategies and occupational health concerns.

  • Coordinate with interdisciplinary teams to ensure continuity of care during discharge planning.

  • Maintain accurate documentation of patient progress, treatments administered, and any changes in condition.

  • Option to travel to various patient locations or 8 to 12 hour shifts 1:1 with adult or pediatric patient

Qualifications

  • Current Registered Nurse (RN) license in the state of Ohio.

  • At least 6 months of clinical nursing experience is preferred.

  • Excellent communication skills to effectively interact with patients, families, and healthcare teams.

  • Ability to work independently while managing multiple patient cases efficiently.
