Responsibilities:

Everyday is different for a CNA. Our Certified Nursing Assistant cares for the ill, injured or convalescing patients or patients with disabilities in a facility. You will be responsible for providing quality healthcare to each patient in adherence with all applicable laws, regulations and policies.

Qualifications:

  • Current CNA licensure with the State Board of Nursing in the State of Colorado.
  • 1 year experience as a CNA in the last 3 years in a supervised setting
  • Current CPR (live demonstration live)
  • Negative TB/PPD or Chest X-Ray within the last 12 months
  • CAPS
  • Successful completion of the skills assessment specific to the areas of experience

The Day Center CNA ( Certified Nurse Assistant) works at our Adult Day Health Center assisting participants with ADLs (activities of daily living) and providing both restorative and supportive care. Follow the care plan and assist participants with daily activities like bathing, dressing, grooming, hair care, mouth care, eating, and toileting. Document care provided in accordance with established policies, guidelines, and applicable regulations. Carry out, reports and documents care given in an effective, timely manner as observed by the RN and through periodic record reviews. Assist participants with restorative and supportive care such as ambulating, positioning, transferring, skin care, catheters, fills oxygen and provides other aides as assigned or needed. Monitor participants health status such as vital signs, weights, skin condition, appetite and changes in functional ability and report pertinent information to clinical supervisor in a timely manner. Collaborate with multidisciplinary teams in care planning for participants. Monitor appetite and look for changes in functional ability and skin condition. Monitor/assist participants in a courteous manner with meals and helps with feeding, as necessary. Report any variance, accident, or unusual occurrence to the appropriate personnel. Assist with loading and unloading of participants from vehicles. Assist the transportation department with participants’ take-home supplies. Serve as a bus attendant on company vehicles while InnovAge drivers transport participants to/from center or medical appointments supporting participants while in transit and accompanying participants in and out of facilities. May rotate into In-Home Services as a Homecare CNA. as deemed necessary by the Center Manager.
