Create, document, and update individualized infusion care plans and maintain thorough, timely documentation in the EHR. Conduct initial patient evaluations for therapy appropriateness, including payer criteria and clinical suitability for infusion center setting. Maintain open, timely communication with medical directors, referring physicians, and interdisciplinary staff regarding care coordination. Identify urgent or emergent reactions during infusion, initiate appropriate interventions, and coordinate emergency transfers when necessary.
