Responsibilities

As a Hospice CNA You will:  

Personal Care: Help with bathing, dressing, toileting, mobility, and repositioning

Monitor & Report: Observe and report changes in the patient’s condition

Mobility: Assist with ambulation, transfers, ROM exercises and safe equipment use

Medication Assistance: Help with self-administered medications per state regulations

Emotional Support: Offer companionship and comfort to patients and families.

Light Housekeeping: Change linens, assist with meals, and maintain a clean space.

Documentation: Maintain accurate and timely documentation

Team Participation: Collaborate with hospice interdisciplinary team

Qualifications

Meet the training requirements of the state of practice
Minimum of six months experience including providing personal care
Must be 18+ years of age
Complete a minimum of 75 hours of classroom and supervised practical training, 16 of the 75 hours of classroom must be complete prior to a mandatory 16 hours of practical training
Possess and maintain current CPR certification
Ability to read and follow written instructions and document care given
Understands hospice philosophy, comfortable providing specialized care to the terminally ill
Ability to work with little direct supervision
Ability to handle death/dying
Strong oral and written communication skills
Good Organizational Skills

Measures and documents blood pressures, TPRs (Temperature, Pulse, Respiration), I & O (Intake and Output), daily weights, and other patient data as requested. Documents accurately in Electronic Medical Record (EMR). Performs purposeful rounding under direction of RN and promptly communicates observations, concerns, and needs of patient/family to RN. Documents appropriately. Performs various patient positioning and transporting duties which require lifting and pushing/pulling while utilizing proper technique and assistive devices. Performs tasks related to nutritional needs of patient under direction of RN to include diet review, tray set up/removal, proper positioning and diet selection. Performs tasks related to maintaining skin integrity to include proper repositioning and skin care. Ensures special patient supplies and appliances are provided and maintained such as TED hose and sequential compression devices. Under the direction of RN and as determined by site/unit requirements, may be required to remove IVs/urinary catheters, simple wound dressing changes, cross train to tele tech, provide post mortem care, perform phlebotomy, ECGs, bladder scans and other skills as allowed under state regulations. Practices within policies and procedures of infection prevention by demonstrating hand hygiene and knowledge of sterile and aseptic procedures, proper disposal of contaminated materials and utilizing proper attire when in direct contact with patients in isolation. Serves as a resource and subject matter expert in a relevant area within department. Trained and competent as Preceptor. Assists with development/ evaluation of orientation materials. Regularly identifies, designs, initiates, and assists in implementing process changes to improve the department, quality and safety of the patient experience. Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served.
