Nurse Practitioner (NP)

As an In-House Nurse Practitioner, you will be the primary provider of internal medicine services within a Skilled Nursing Facility (SNF). You will have the opportunity to build strong, lasting relationships with residents, their families, facility staff, and providers, while being an integral part of the clinical leadership team.

At Trio Health Partners, we focus on preventing critical health issues before they arise, delivering proactive care to residents in our Skilled Nursing and Long-Term Care facilities.

Job Requirements:

  • Active Illinois Nurse Practitioner License (unencumbered).

  • Master's Degree in Nursing from an accredited program.

  • Strong clinical background in primary care and preventive medicine.

  • Geriatric or adult medicine experience preferred, with a focus on skilled nursing, long-term care, hospice, palliative care, or similar settings.

  • Ability to engage and communicate effectively with elderly and chronically ill patients and their families.

  • Ability to meet a minimum of 12 patient visits per day, with bonuses for surpassing this target.

  • Knowledge of Medicare, Medicaid, and Health Plan benefit structures is a plus.

Key Responsibilities:
As the In-House Nurse Practitioner, you will provide essential care and leadership within the facility, including:

  • Conducting new admission evaluations and monthly stability visits for long-term residents

  • Performing comprehensive physical examinations and evaluating changes in patient conditions

  • Prescribing medications and treatments as needed

  • Developing patient care policies and facilitating referrals to specialists

  • Coordinating advanced care planning and discharge visits

  • Ensuring timely communication and documentation of patient assessments, treatment plans, and follow-ups

  • Supporting the physician-patient relationship and addressing gaps in care

Our NPs work closely with the interdisciplinary team, taking a leadership role in patient care, collaborating with physicians, and ensuring high-quality service delivery in our facilities.

Salary & Compensation:

  • Annual Pay: $105,000 - $130,000 based on experience

  • Monthly Bonus Opportunities: Monthly performance-based bonuses
