Responsibilities

Performs initial nursing evaluation, including behavioral health/psychiatric nursing needs.
Observes and evaluates the physical and mental status of the patient for complications and medication side effects.
Maintains awareness for indicators in patients' behavior that call for change in the treatment modality (crisis, suicidal ideation) as well as other medical signs and symptoms.
Instructs patient/caregiver in cognitive and technical aspects of care, medication regimes, disease processes, safety interventions, ADL care, coping skills, symptom management and other interventions.
Administers IM medications, performs lab draws and specimen collections.
Acts as a case manager in coordinating care for patients as assigned.
Completes documentation timely and accurately.
Supervises LPNs and HHAs.
Regularly communicate patient progress to the clinical manager, psychiatrist and care team.

Qualifications

Current RN license, specific to the state(s) you are assigned to work. Must have one of the following to meet CMS requirements for psychiatric nursing in the home care setting:
- An RN with a master's degree in a mental health nursing or psychiatric specialty. Must have nursing experience in an acute treatment unit or other psychiatric setting.
- An RN with a bachelor's degree in nursing. Must have one year of recent nursing experience in an acute treatment unit or other psychiatric setting.
- An RN with a diploma or associate's degree in nursing. Must have two years of recent nursing experience in an acute treatment unit or other psychiatric setting.

Please note: Experience must have been within the last three to five years. If not, documentation must support psychiatric retraining or classes or CEUs to update psychiatric knowledge.

Current CPR certification.
Valid driver's license, reliable transportation and liability insurance.

This role is a 1099 contract opportunity, with care being provided at the patient’s residence. Patients range from weekly hours to 24-hour care. This allows for multiple scheduling options, from short visits on a weekly basis to full 12 hour shifts or a blend of both. Patients range from high acuity to those who only need basic medical management. Roles and Responsibilities: · Provides and coordinates care by assessing physical and behavioral health needs of client, develops and implements nursing care plans, maintains medical records and educates clients and their families about various physical and behavioral health conditions while ensuring client safety at all times. · Assesses client’s status on an ongoing basis and pertinent information gathered is documented. · Ensures nursing treatment plans are specific to assessed needs and initial treatment plan is completed upon admission. · Takes physician’s orders (verbal or via telephone) accurately and documents appropriately; transcribes and reviews physician’s orders. Attends and participates in the unit activities such as therapy groups, family meetings as appropriate, staffing, unit meetings, etc. · Provides education/explanation to clients, families, and co-workers regarding medication usage/maintenance, common side effects, client's rights, as well as other topics necessary for optimal client care. · Monitors client acuity, census, staffing needs, and makes arrangements for coverage as needed, following established guidelines. · Assures thorough, accurate documentation is completed for all clients on each shift and client assignment sheets are completed daily and tasks are carried out as assigned. · Gives thorough report to oncoming staff, supervisors, and physician. · Provides coverage in emergency services as needed. EDUCATION/EXPERIENCE/SKILL REQUIREMENTS: · Graduation from an approved/accredited school of nursing. · One or more years’ nursing experience within clinical area of service provided by facility preferred. · Experience in behavioral health/psychiatric/addiction recovery environment preferred.
