Job Summary

As a Care Manager RN, assess and organize patient care through treatment stages, following guidelines. Manage benefits, oversee services, act as a case manager and trainer to achieve optimal results. Effective care management helps minimize unnecessary resource use.

Essential Functions of the Role

  • Conduct thorough assessment, implement care plan, collaborate with team for evidence-based high standard of care.
  • Creating patient care plans that focus on interventions, treatment goals, and self-management for the patient's benefit.
  • Examine the health needs of patients and families, then provide necessary care to meet those needs.

JOB SUMMARY The Registered Nurse (RN) Unit PRN is a licensed professional who uses the BSWH nursing professional practice model to coordinate patient care delivery by the health care team. Job Summary and Qualifications As an Emergency Room Registered Nurse (RN), you will be responsible for performing assessments, developing a plan of care, and implementing and evaluating care for assigned patients. You will collaborate with other healthcare disciplines to refine and achieve plans of care, and will continually monitor the patient’s physical condition, emotional and social needs.
