Position Summary: LPN – Licensed Practical Nurse

As an LPN at Charter Senior Living, you are a cornerstone of compassionate care in our Assisted Living and Memory Care programs. You’ll provide hands-on nursing support, medication administration, care plan implementation, and leadership to ensure each resident enjoys a safe, fulfilling, and meaningful life.

You’ll also play a key role in promoting teamwork, mentoring caregivers, and maintaining strong communication with residents, families, and staff.

Key Responsibilities

  • Administer medications and treatments in accordance with state regulations and company standards

  • Conduct resident health assessments and contribute to individualized care plans

  • Monitor resident condition changes and communicate promptly with leadership and families

  • Supervise and support caregivers and CNAs to ensure quality care delivery

  • Document all nursing services, assessments, and interventions accurately

  • Uphold infection control and safety protocols to protect residents and staff

Qualifications

License: Current LPN license in the State of Kentucky (required)
Experience: Prior nursing experience in senior living, long-term care, or memory care preferred
