Hours: Monday-Friday 8:30-5

 

The Advanced Practice Nurse will be responsible for, but are not limited to:

  1. Conduct visits to patients on palliative care in accordance with local, state and federal regulations.
  2. Communicate visit findings to designated members of the interdisciplinary team, including the National Director of Palliative Care.
  3. Facilitate access to appropriate support models of care (i.e., hospice, rehabilitation, on-going aggressive treatment).
  4. Consult with other members of interdisciplinary team to contribute to the patient's plan of care.
  5. Prepare and maintain accurate patient records, charts and documents.
  6. Assist in development of clinical practice guidelines/standards in support of quality palliative care as needed.
  7. Ensure collaboration with patients, caregivers, medical directors and members of IDT.

We primarily work weekends, with some occasional weekdays under the Reserve Health Readiness Program (RHRP) in the State of Kansas on an "as needed basis".
