At Home Care Partners is recruiting RNs to perform in home assessments of individuals who require “just enough” supportive and personal care to enable them to remain in the comfort and security of their preferred environment, home.

Responsibilities:

Responsibilities include assessment of individuals needing care; development of an individualized care plan; and orientation and supervision of the personal care / home health aide team

Part Time/ Hourly work schedules available! Work an average of 10 to 15 hours / week.; typically week days & during the day time hours

Requirements:

Strong, compassionate and collaborative communication skill; ability to maintain a flexible schedule; and, reliable vehicle & valid NYS driver’s license.
