Family Medicine/Primary Care - Nurse Practitioner

Coordinates and provides care for a group of patients spanning all age groups and cultures for a designated time frame using therapeutic means, the laboratory process, the environment/instrumentation and other health care team members while assuming responsibility and accountability for these patients; responsibilities are to provide patient care in an ambulatory care setting under the clinical supervision of a physician or MD within the guidelines of nursing process and nurse practitioner protocols as outlined in Patient Care Guidelines for Nurse Practitioners.

Report to: Medical Director and Practice Manager (Administratively)

Coordinates Assignments, Workflow, and Activities/Provides for Guidance: Medical Assistant

Required Skills/Qualifications/Training/Experience:

  • Graduate of an accredited school, master prepared with a degree as a Nurse Practitioner Massachusetts licensure.
  • Departmental specialized training to provide care to patients within role and successful completion of SMH orientation program

Preferred Skills/Qualifications/Training/Experience:

  • Command of verbal and written English
  • Operation of clinical equipment demonstrates clinical competence,
  • Positive interpersonal communication skills and good organizational skills
  • Three to five years medical office experience

Educational Requirements: Master in Science in Nursing/Nurse Practitioner

License/Certification: CPR certification. MA NP license, DEA & Mass Controlled Substance Registration

You will have such duties and responsibilities as are set forth below and as may be reasonably requested by SSH from time to time. Essential Functions: Obtains a detailed and accurate medical history and performs a comprehensive physical exam for patients admitted to the Hospital. Orders necessary diagnostic tests, consultations and procedures as indicated or requested by the Hospital Medicine Consultant or attending physician. Identifies and manages chronic medical conditions and provides continuous care to hospitalized inpatients. Has familiarity with current guidelines regarding the assessment of the risk of a perioperative cardiovascular event for pre-operative patients (such as RCRI, and ACS-NSQIP) and pre-operative medical optimization. Prescribes medications, as necessary. Writes orders in accordance with current practice guidelines. Writes complete and comprehensive notes on all patients using appropriate electronic systems consistent with Hospital policy. Facilitates timely care, as necessary. Actively participates in the South Shore Health System readmission reduction efforts. Reviews the care of each patient with the attending physician and other consultative services. Maintains communications with referring physicians. Collaborates with the patient care team (physicians, nurses, PT/OT, case management) to develop and facilitate care plan.
