Role Purpose

The Nurse Practitioner (Hospitalist) utilizes a high degree of knowledge, skill, and competence to deliver inpatient care in a hospital setting. This position assesses, diagnoses, and initiates a patient care treatment plan within the scope of a formal physician collaborative practice agreement. In this position, patient care is performed with considerable independence, guided by physician-approved protocols, and evaluated through chart review, consultations, and the observation of results under the general medical direction of their collaborating Hospitalist physician.

The ability to work in a constant state of alertness and in a safe manner is an essential function of this job.

In addition to resident care, the Nurse Practitioner provides bedside education for staff. The Nurse Practitioner works in collaboration with the community Medical Director in providing care and services to residents; Practices within the scope of licensure and in accordance with the state Nurse Practice Act and state and federal licensure rules and regulations for the level of care in which they practice; Practices in accordance with EverTrue clinical and administrative policies and procedures and in accordance with the Collaborative Practice Agreement . Responsibilities 1. Performs physical examinations and institutes preventive health measures 2. Orders, interprets, and evaluates diagnostic tests to identify and assess resident’s clinical problems and health care needs 3. Records physical findings, and formulates plan and prognosis, based on patient's condition utilizing electronic health records. Maintains medical records and submits at least 10% of records to the Medical Director for bi-weekly review in accordance with the “Collaborative Practice Agreement” 4. Records all services performed for accurate billing for services provided in compliance with all local, state and federal laws, as well as payor rules and regulations; Submits billing documents to billing agency in a timely fashion 5. Works with the Medical Director, other in-house physicians, staff, residents and family members to develop a comprehensive resident care plan and reviews plan periodically with resident’s physician 6. Prescribes, recommends, or discontinues drugs and treatments in accordance with the “Collaborative Practice Agreement” and the state’s “Nurse Practice Act”; Assists EverTrue in establishing formularies for medication and other therapeutic measures 7. Prescribes therapy such as physical therapy, respiratory therapy, or related therapeutic procedures 8. Refers patients to Physician for consultation or to specialized health resources for treatment with a goal of achieving better outcomes in multi-disciplinary treatment plans; Certifies and re-certifies resident for Medicare stays 9.
