The Clinical Director of Nursing is responsible for the overall direction, coordination, and management of all clinical hospice services. This role ensures that quality, safe, and compliant care is provided to patients and families in accordance with the philosophy, mission, and policies of Swan Hospice, as well as all federal, state, and accreditation requirements.
Essential Duties & Responsibilities
Direct, supervise, and evaluate the performance of clinical staff including nurses, aides, social workers, chaplains, and therapists.
Ensure compliance with all CMS, state, and accrediting body regulations governing hospice care.
Participate in the development and implementation of policies, procedures, and best practices.
Oversee patient care delivery, ensuring individualized and compassionate plans of care.
Lead Interdisciplinary Team (IDT) meetings, fostering communication and collaboration among staff.
Monitor quality improvement initiatives and patient/family satisfaction.
Manage staffing assignments to ensure adequate coverage for patient needs across service areas.
Provide direct nursing care as needed in complex or urgent situations.
Collaborate with the Administrator/CEO on strategic planning, budgeting, and program development.
Act as a resource, mentor, and educator for clinical staff.
Ensure timely and accurate clinical documentation in compliance with regulatory standards.
Participate in hiring, onboarding, training, and disciplinary actions for clinical staff.
Serve as the primary clinical contact for physicians, referral sources, and regulatory agencies.
Qualifications
Current Registered Nurse (RN) license in [state].
Minimum of 3–5 years of clinical nursing experience, with at least 2 years in hospice, palliative care, or home health.
Prior leadership/management experience required.
Strong knowledge of Medicare/Medicaid hospice regulations.
Exceptional organizational, leadership, and communication skills.
