POSITIONOVERVIEW:
TheCCBHCiscomprisedofseveralcommunitiesbasedprogramsprovidingdirectcaretoover3,200clientsannually,bothonsiteandinthecommunity.Aregisterednurse(RN)assignedtothefacilityisresponsibleforscreeningconsumersforBMI,regularscreeningofA1Clevelsforconsumersusinganti-psychoticmedications,andperformanceofallroutinemedicalprocedureswithinthescopeofthenursingpracticefortheclientsunderhis/hercharge.
KEYESSENTIALFUNCTIONS(Listthefunctionsthatareessentialtoachievethegoals):
- Performnursingassessmentoneachclientupontheclient'sentryintotheprogramand(atminimum)annuallythereafter.Thepurposeoftheassessmentistodeterminetheclients'generalhealthcondition,identifyanyriskfactorswhichmayexist,andprovidecareforanypresentingorout-standingmedicalissues.Attheconclusionofsuchanassessment,theRNmayrefertheclienttoaphysicianforfurtherfollow-uptreatment,ifrequired.
- Conductsubstanceabuseassessmentstoascertainclients'needsfortreatment.
- Developatreatmentcareplanbasedonclients’needs.Ensurethatanynecessaryfollow-upwilloccurandanynecessaryorprescribedcareneedisadministered.
- Inlinewithdoctor’sorders,administerprescribedand/orroutinemedicaltreatments.EnsurethatallNYStateRegulationsregardingsuchadministrationareadheredto.
- Assistclientswithschedulingclinicappointmentsandaccompanytheclientwhenrequired.
- Functionasaclinicalliaisonbetweenexternalmedical/psychiatrichealthcareproviders,schools,othercommunityorganizationsandthefacilitywithregardstotheclient'smedicalneeds.
- Functionasaclinicalliaisonwithinaninterdisciplinaryteamofmedical/psychiatricproviders,socialworkers,carecoordinators,casemanagersandpeers.
- Responsibleforhealtheducationandcommunicatinginformationregardinggoodhealthpracticestotheclient'sunderRN'scare.
- Mayrunmedicationgroups.
- Respondtomedicalemergenciesimmediatelyuponnotification.Performwhatevertriageisnecessarytostabilizetheclientandprovideaccesstoallnecessarymedicaltreatment.Followuptoensurecompleterecovery.
- Performallnecessarychartingandotherpatientdocumentationasrequiredbyinternalpracticeandexternalregulatorybodies.
- Obtainlabspecimens,orderedbyprescribers,whichmayincludespecimenpreparationbeforesendingtothelab.
- Monitorlabresultsforanyindicationsofchange;communicatesuchchange(s)tothephysician.
- Provideadvocacyforhospitalizedclients.
- WillcomplywithallOSHArulesandguidance.
- Willparticipateinanyandallprogramreviewsbothinternalandexternal.
EDUCATIONAL/TRAININGREQUIRED(Listallthatisrequiredtoachievethisposition):
- RN,BApreferred,innursing.NewYorkStateregistrationisrequired.
- Experienceinthementalhealth/substanceabusefieldstronglypreferred.
VISUAL AND MANUAL DEXTERITY:
- The candidate should be able to read documents for analytical purposes such as health charts, spreadsheets, etc.
- Possess the manual dexterity necessary for handling medical equipment, administering medications, and performing clinical procedures.
WORK ENVIRONMENT/PHYSICAL EFFORT
- Ability to walk, sit, stand, and bend as necessary to assess and provide care to clients effectively in various settings.
- Must be capable of responding swiftly to emergencies, which may involve moving quickly to the scene, performing CPR, and utilizing appropriate emergency protocols.
- Proficient in lifting, transferring, and repositioning clients safely when needed, ensuring the comfort and safety of both clients and staff.
- Maintain the physical stamina to perform nursing duties throughout the shift, including prolonged periods of standing and walking, while remaining attentive to patient needs.
- Ability to navigate a variety of environments, including tight spaces, while maintaining a focus on patient care and safety.
