As a Hospice Admissions RN You will:    

  • Visit patients (in homes, hospitals, nursing facilities, etc.) who are referred for hospice care 
  • Explain hospice services, discuss goals and care with the patient and families, answer any questions, and obtain necessary consents and legal documents 
  • Obtain medical history, current medications, and symptoms 
  • Perform comprehensive clinical assessments to determine eligibility for hospice based on Medicare/insurance guidelines 
  • Coordinate the start of services (nursing, aides, equipment, medications.) 
  • Collaborate with interdisciplinary team 
  • Maintain accurate and timely documentation

Qualifications

  • Graduate of an accredited nursing school with current licensure to practice in state of operation 
  • One year of hospice nursing experience required, 3+ years preferred or similar setting such as Long-Term Care, Intensive care, or Emergency room will be considered in place of hospice nursing experience 
  • Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order

Notify the facility pharmacy and the attending physician promptly of new or changes to orders. Safely and accurately prepare and administer medications and perform treatments as ordered by the physician. Assess, monitor, and evaluate residents’ physical and emotional well-being throughout your shift, reporting any significant status changes to the Director of Nursing and physician immediately. Supervise Licensed Practical Nurses (LPNs) and Certified Nursing Assistants (CNAs) on your unit, ensuring the highest standards of care are delivered. Participate in the interdisciplinary care team to develop and review personalized resident care plans. All other clinical and administrative duties as assigned by the Director of Nursing.
