Nurses are our greatest assets! Clinical Supervisors directly communicate with, supervise and educate our nurses in the field to ensure they feel connected and supported by our in-office team. They provide overall management support and supervision of all client care activities within the home health setting. Clinical Supervisors also perform case management and serve as patient and family advocates. Working in conjunction with the interdisciplinary team, the Clinical Supervisor helps to coordinate care to meet patient goals and maximize the patient care delivered by our field team They revise and implement the plan of care to ensure every patient is receiving the best possible care as directed by their physician. Strong and effective communication skills are imperative to a successful relationship between supervisors and families.

The responsibilities of this job include, but are not limited to, the following:

  • Responsible for coordinating care for assigned pediatric and adult patients, and establishing a goal directed care plan from admission to discharge which includes a comprehensive ongoing assessment of clients’ needs
  • Care management which includes performing on site supervisory visits to assess client, family, environment, and clinical care givers and complete follow-up documentation
  • Ensure availability and proper operation of necessary equipment and supplies related to patient care
  • Provide ongoing supervision, orientation, training, education, and evaluation of clinical field staff
  • Ensure nursing practice in accordance to the Physician’s orders.
  • Evaluate the quality and effectiveness of nurse practice and nursing services, analyzing appropriate data and information to identify opportunities for collaboration with all stakeholders in order to improve services and patient outcomes
  • Identify professional practice standards within the organization and identify areas of strengths as well as areas for professional practice development
  • Maintain compliance in accordance with company policies and procedures, laws and regulations, and professional standards within the state of practice
  • Maintain a professional demeanor consistent with registered nurse standards of practice
  • Base decisions and actions on ethical principles and foster a non-judgmental, non-discriminatory climate in which care is delivered in a manner sensitive to socio-cultural diversity

Minimum education and professional requirements include, but are not limited to, the following:

  • Diploma, Associate, or Bachelor degree in nursing from state accredited RN program (REQUIRED)
  • Active PA Registered Nurse License (REQUIRED)
  • Valid Driver’s license and Acceptable MVR (REQUIRED)
  • At least one year of nursing experience (REQUIRED)
  • Previous pediatric or home health experience preferred
  • Previous case management experience preferred
  • Supervisory experience preferred
