Acts is currently seeking qualified candidates for the role of Licensed Practical Nurse (LPN) in our skilled nursing neighborhood.

In this role, you will be responsible for providing prescribed medical treatment and personal care services to our residents in skilled nursing. Participates in the supervision of nursing assistants, and coordinates the provision of resident care according to the care plans.

  • Current State Licensed Practical Nurse (LPN) license
  • Current or eligible for certification in CPR

Pay Range

$25.60 - $32.51 / hour. Starting rate will vary based on skills and experience.

The care may include, but is not limited to, the following: obtaining and recording in the medical record: blood pressure, pulse, temperature and pulse oximetry, drawing blood, EKG, administration of oral medications, transporting patients, obtain urine and stool specimens, perform point of care testing, assist with procedures consistent with the scope of the employee’s position (i.e.. dressing changes, dressing, grooming, toileting, ambulating, transferring, observing, and reporting the general well-being of the person(s) to whom they are providing care). Responsibilities: The LPN must read and interpret documents such as doctors’ orders, medication names, and lab tests. Perform focal assessments for established inpatients. Provide general basic nursing care, such as positioning patients, lifting and turning, assisting in use of bedpan, commode, urinal or bathroom, and weighing. Provide exceptional customer service that aligns with the Planetree philosophy. Perform routine, uncomplicated treatment procedures including enemas, application of hot and cold compresses, dressing changes, wound cleansing, obtaining vital signs including orthostatic vital signs and rectal temperatures and splinting of extremities.
