Community Options, Inc. is a national non-profit agency providing services to individuals with disabilities in 12 states.
We are now hiring a skilled Parttime Licensed Practical Nurse in Scotch Plains, New Jersey. The L.P.N. delivers care to individuals in a community setting utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation. This person will interact with individuals, significant others, and health team members while maintaining the standards of professional nursing.
Pay Rate: 30.00 per hour
Responsibilities
  • Provide treatment and health care procedures ordered by a physician and required by the standards of professional practice or law
  • Ensure individuals attend medical appointments per their Individual Support Plan (ISP)
  • Advise supervisor of all concerns and issues related to the provision of nursing services
  • Develop and update Individual Support Plans (ISP)/Person Centered Plans (PCP) with an interdisciplinary team to include any concerns, observations, and behavioral developments
  • Train employees on the medical needs of assigned individuals
  • Monitor health and medical conditions of individuals and refer for medical services as needed
  • Monitor the use of medications through medication administration logs
  • Ensure program documentation and billable records are completed accurately and timely
  • Build a support network for individuals by developing and maintaining relationships with community members, businesses, and families
  • Additional tasks and responsibilities may be assigned
  • Employee must cooperate with the license and department staff in any inspection, inquiry, or investigation
Minimum Requirements
  • Bachelor’s degree or equivalent experience in nursing field
  • Minimum of one year of experience providing nursing services to individuals with disabilities
  • Valid nursing license
  • Valid driver’s license with a satisfactory driving record
  • Ability to calculate figures related to medication dosages
  • Ability to establish a comfortable and supportive relationship with an individual receiving supports
  • Ability to effectively present facts and recommendations
  • Employment is contingent upon successful completion of checks of criminal background, central registry, child abuse registry, and drug testing

Purpose of Your Job Position The primary purpose of your position is to provide direct nursing care to the residents, and to supervise the day-to-day nursing activities performed by CNAs/GNAs and other nursing personnel. To monitor the performance of CNAs/GNAs, nursing and non-licensed personnel, provide education and counseling, perform disciplinary action as necessary, and complete performance evaluations. Such supervision must be in accordance with current federal, state, and local standards, guidelines, and regulations that govern our Center, and as may be required by the Director of Nursing Services or Nurse Supervisor to ensure that the highest degree of quality care is maintained at all times. Duties and Responsibilities Administrative Functions Direct the day-to-day functions of the CNAs/GNAs or other nursing personnel in accordance with current rules, regulations, and guidelines that govern the long-term care Center. Ensure that all assigned nursing personnel including CNAs/GNAs comply with the written policies and procedures established by this Center. Periodically review the department’s policies, procedure manuals, job descriptions, etc. Make recommendations for revisions. Provide discipline and evaluations of assigned CNAs/GNAs and other nursing personnel in accordance with current rules, regulations, and guidelines that govern the Center. Issue verbal and written disciplinary warnings to assigned CNAs/GNAs and other nursing personnel for violations of current rules, regulations, and guidelines of the Center. Meet with your assigned nursing personnel, including CNAs/GNAs, as well as support personnel, in planning the shifts’ services, programs, and activities. Interpret the department’s policies and procedures to personnel, residents, visitors, and government agencies, as required. Admit, transfer, and discharge residents, as required. Complete accident/incident reports, as necessary. Provide information to the Billing Office as necessary to insure accurate resident billing. Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc., as necessary. Strong knowledge of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to nursing care centers. Provide coaching, development, training and performance evaluations to assigned nursing and nursing support staff. Participate in the development, maintenance and implementation of the Center’s quality assurance program for the nursing service department.
