Your Responsibilities:

Provide resident-centered care, assisting with physical, cognitive, and psychosocial needs
Administer medications and treatments accurately while monitoring effectiveness and documenting as per policy
Deliver specialized nursing care for residents with therapeutic, neurological, dementia, dialysis, IV therapy, infectious disease, and wound care needs
Ensure compliance with facility policies, state regulations, and resident safety protocols
Collaborate with the interdisciplinary team to implement individualized Care Plans
Perform additional duties as assigned to support residents and facility operations

Qualifications:

Graduate of an accredited nursing program
Active LPN license in Pennsylvania (in good standing)
Strong clinical, communication, and problem-solving skills
