Qualifications as a Licensed Practical Nurse LPN 

  • PA LPN License
  • Complete individualized patient care plans and perform care management and care coordination services 
  • Contact patients to provide care coordination, support, and manage compliance with the care management programs 
  • Ability to Adapt and take on all responsibilities of the Licensed Practical Nurse LPN role
