Assists residents to attain and maintain optimal functional status by contributing to the development and implementation of the nursing care delivery system. Ensures care meets all standards of care as set forth by The Enclave at Cambridge while working under the supervision of a Registered or Licensed Practical Nurse.

Primary Job Duties:

•Follows established Resident’s individualized plans of care under the general direction of the Charge Nurse.

•Provides care which may include, but not limited to: •Mobility functions •Grooming functions •Dining functions •Toileting functions

•Follows established detailed individualized restorative STNA functions under direction of nursing.

•Documents all care, interventions and events in patients record.

•Follows infection control principles, policies and procedures.

•Communicates changes in resident conditions or treatments to RN supervisor and appropriate disciplines.

•Supports positive relationships.

•Insures a safe and comfortable environment for residents, staff and visitors at all times.

•Immediately report suspected or actual abuse, mistreatment, neglect, exploitation, and misappropriation of property to the CEO/Administrator in accordance with the policy set forth in the employee handbook.

•Immediately report suspected or actual injury received while in the commission of my position to my immediate supervisor.

•Supports and maintains quality assurance, residents’ rights and safety regulatory standards.

•Performs other duties as assigned by the Director of Nursing/Supervisor and Administration. Health Caring Standards/Values:

•Greet residents, visitors and co-workers immediately with a smile, warm greeting, and introduction, calling them by name if possible.

•Anticipate the unexpected and exceed the expected by asking “Is there anything else I or a fellow co-worker can do for you”.

•Acknowledgement for the need of collaborative efforts with the elderly, their family and friends, and the larger community to provide a life-giving environment that supports the Ethical and Religious health care directives of Astoria Place Excellence, Professionalism, Integrity, and Compassion.

Skills and Abilities: Must have a working knowledge of quality, of both Short Stay and geriatric nursing care. Good verbal and written communication and the ability to work as a member of a multidisciplinary team. Must maintain confidentiality and the ability to prioritize to ensure good time management.

Education and/or Experience: Completion or enrollment in State Approved Nurse Aid Training and Competency evaluation program; High school Diploma or G.E.D. preferred, not required. Previous experience in working in a nursing home/long term care and in working with elders preferred but nor required; Graduation of State Approved Training Program and successful completion of Nurse Aide examination to be a State Tested Nurse Aide preferred.
