Job Summary

  • The delivery system practiced at FHN Memorial Hospital employs the concepts of differentiated nursing incorporating the PRIDE Standards as set forth by FHN.
  • Provides direct nursing care to identified patient population.
  • Delivers direct patient care to achieve optimal patient outcomes.
  • Demonstrates competencies in accordance with specific performance standards.

Job Responsibilities

  • Other duties may be assigned.
  • Demonstrates clinical competency in providing age specific care to patients in designated areas of practice.
  • Performs therapeutic measures and assists with ADL’s.
  • Assures accurate and timely medication administration.
  • Assists with completion of shift assessments.
  • Evaluates, reports and documents patient outcomes.
  • Assists with patient education and psychosocial needs.
  • Assists with discharge instructions.
  • Delegates appropriate tasks to unlicensed staff Responds to customer requests/concerns.
  • Performs phlebotomy on all age groups.
  • Displays competency for the Licensed Practical Nurse role as evidenced in the annual performance appraisal following the FHN PRIDE Standards.
  • Monitors and evaluates the work of unlicensed staff.

Qualifications


Requirements

Education:  Graduate from an accredited LPN program
Required Experience:   Zero to six months related experience and/or training or equivalent combination of education and experience.

Licensure/Certification/Registration:  Current licensure as an RN/LPN in Illinois; CPR certification; Pharmacology (will be provided if not present at hire).

May chart on behalf of the provider when in the exam room. Chart note to include name, title, date and time of the entry. Chart must then be signed off on by the provider. Assigns ICD-10 codes on charge sheets or in EMR for billing office and monitors CPT coding to assure proper documentation exists for the physician. Performs other duties as assigned by supervisor. SPECIALIZED KNOW-HOW & REQUIREMENTS: Current Illinois Practical Nursing Licensure required. Previous Medical Office experience preferred. BLS required within 30 days of hire and must be maintained throughout employment at CGH. Thorough knowledge of medical equipment and instruments is strongly preferred. Prior knowledge in drawing blood preferred. Basic computer knowledge preferred. Demonstrates ability to document patient needs and assessments. Demonstrates knowledge necessary to provide care appropriate to any age-related needs of the patients served in the physician office. Demonstrates knowledge of the principles of growth and development appropriate to the patient population served. Able to assess and interpret data about the patient’s status in order to identify each patient’s needs and provide the appropriate care, including age-related care, needed by the pediatric, adolescent and/or geriatric patient group.
