Description

The Psychiatric Nurse Practitioner will be a member of the Interdisciplinary team, whose purpose is to evaluate, treat and plan for the aftercare of patients.


Responsibilities

  1. Provide direct care to patient assigned to the team, including appropriate and timely care and documentation.
  2. Complete admission assessment, with a focus on comprehensive risk assessment for all new intakes, and reassessment of patients in a timely manner consistent with policy.
  3. Conduct appropriate outreach and relevant documentation as per policy.
  4. Complete treatment plan documentation.
  5. Participate in all huddles to share information.
  6. Ensure appropriate use of de-escalation, participate and provide leadership in behavioral codes.
  7. Participate in psychiatric codes, medical codes in the clinic until code team arrives.
  8. Work with all teams to help patient move through appropriate level of care.
  9. Appropriate risk stratification and re-assessment of risk level.
  10. Participate in discharge planning and including referrals to alternative levels of care.
  11. Collaborate and coordinate with primary care and other medical specialists.
  12. Participate and conduct case reviews.
  13. Work closely with the collaborative physician.
