Responsibilities
Provide skilled nursing interventions in accordance with the physician’s orders and individualized care plan (medication administration, wound care, respiratory care, seizure support, etc.)
Complete nursing assessments at the beginning of each shift and document observations clearly in client records
Follow infection control protocols and report any signs of illness, abuse, neglect, or changes in condition to the agency and physician
Perform catheterization, suctioning, enteral feeding, and medication administration via G-tube/J-tube with excellence and safety
Submit timely electronic clinical documentation using the agency’s secure system (Notion or equivalent HIPAA-compliant platform)
Coordinate with agency RN supervisors for scheduled supervisory visits and plan-of-care reviews
Adhere to IDPH, Medicaid/Medicare, and DSCC care documentation, licensure, and training compliance requirements
Educate and support families and caregivers in-home to ensure continuity and confidence in care
Participate in emergency preparedness plans and trip protocols as needed
Maintain up-to-date CPR/BLS certification and complete required CEU and annual training hours as mandated by IDPH (77 Ill. Adm. Code 245.70)
