The Lead RN serves as the primary clinical resource for RNs, LPNs, Social Workers, and Medical Assistants, to drive both clinical excellence and operational efficiency. Responsibilities & Duties: The Lead Registered Nurse is primarily responsible for: Provide day-to-day clinical supervision for RNs, LPNs, and Social Workers within the cardiovascular VBC program. Ensure adherence to evidence-based guidelines for heart failure, CAD, hypertension, AFib, and post-MI care. Review and manage escalations from RPM alerts (e.g., weight gain, uncontrolled BP, abnormal HR). Oversee Transitional Care Management (TCM) patients to ensure timely outreach and follow-up. Conduct post-hospital discharge outreach to prevent readmissions. Audit care minutes completed in CCM dashboards and RPM (16/20 compliance checks). Lead quality metrics and performance tracking; report outcomes to leadership and address underperforming measures. Train RNs and MAs on documentation workflows, quality standards, and program protocols. Onboard and train new staff (RNs and MAs) on cardiovascular workflows, documentation, and RPM processes.
