Sign on bonus! At Tucker House Nursing and Rehabilitation Center the Unit Manager assumes responsibility and accountability for the nursing care of all residents on an assigned unit. Supervises licensed nurses and other nursing personnel in the delivery of care and services according to each resident’s comprehensive resident assessment and plan of care. The incumbent will also perform other duties as outlined below: •Manages unit in accord with facility philosophy, in compliance with standards of professional nursing practice, and with all state and federal laws and regulations. •Utilizes resources in an efficient and economic manner to ensure that each resident achieves the highest practicable physical, mental and psycho-social well-being. •Makes daily rounds on unit to supervise, observe, examine, interview residents, to evaluate staffing needs, to monitor regulatory compliance, to achieve the care environment and to evaluate staff interactions and clinical skills competency • Supervises nursing staff in the implementation of plans of care to assist residents to attain or maintain highest practicable physical, mental and psycho-social well-being •Audits 24 hour report as completed by Charge Nurses and reviews clinical records to identify problems, to ensure timely responses to residents' needs, to evaluate documentation skills of unit nursing staff, and to evaluate regulatory compliance with documentation requirements •Listens to verbal shift report making special note to follow up on significant changes in condition, admissions, transfers, discharges, initiation or use of physical or chemical restraints, unnecessary use of any drug, incidents, unexplained injuries, medication errors, loss of resident property or evidence of resident or family dissatisfaction. •Assists Charge Nurses with the assignment of nursing staff to ensure sufficient number of staff. Evaluates staffing patterns based on unit census, resident acuity, and other resident needs. Adjusts staffing pattern accordingly. Communicates difficulty meeting resident needs and maintaining regulatory compliance promptly to Director of Nursing Experience: 1-3 year professional nursing experience providing direct care in long term care facility, with 1 year supervisory or management experience with direct care staff. Education: Graduate of accredited school of nursing. Certification: CPR Certification and Current active license as RN in Pennsylvania

Tucker House Nursing and Rehabilitation is now hiring a Nurse Assessment Coordinator (RNAC). Registered Nurses, Sign on Bonus! The MDS Coordinator is responsible for the timely completion of all OBRA and Medicare assessments to comply with federal regulations. Oversees the overall process and tracking of Prospective Payment System (PPS) Minimum Data Set (MDS) assessments including completion, submission and correction as needed. Job Responsibilities: Integrates nursing, dietitian, therapeutic recreation, restorative, rehabilitation, and physician services to ensure accurate assessments which capture all clinical conditions and provided services. Confirms payer source of all residents per HIQA. Ensures the accurate and timely completion of all MDS Assessments including OBRA and Medicare PPS. Maintains an accurate schedule of all MDS assessments to include the proper reference dates throughout the resident’s stay. Responsible for electronic submission of all MDS assessments. Verifies electronic submission of MDS and performs corrections when necessary and maintains appropriate records. Coordinates interdisciplinary participation in completing the MDS for each resident according to regulatory time frames. Ensures completeness of MDS and thoroughness of documentation as mandated by federal and state standards. Schedules resident care conferences in compliance with state and federal regulations and ensures completion of all MDS reviews prior to resident care conference Assists disciplines with care planning process by ensuring the MDS information is completed timely, the Care Area Assessments (CAAs) are analyzed thoroughly, and the team has completed the MDS prior to care conference. Ensures all Medicare documentation follows federal and state guidelines and is completed timely. This includes the physician certification/Recertification, Traditional Medicare and Medicare Advantage non-coverage letters for part A and B. Assess and evaluates the outcomes of the RAI process to determine additional training, education and monitoring needs Validates that the documentation supports MDS coding. Collaborates with the RAI Specialist(s) and the Interdisciplinary team to identify and capture opportunities to assist residents in reaching their optimal physical and psychosocial levels. Job Qualifications: Education: Graduate of accredited school of nursing. Certificate/License: Current license as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) in PA. Work Experience: One year of clinical experience. Experience with MDS completion, reimbursement, clinical resource utilization is highly desirable.
