JOB DUTIES: The purpose of your job as a Licensed Practical Nurse is to perform a variety of care practices under the direction of the Registered Nurse and/or Supervisor of the unit/shift. This includes, but not limited to: Take and record measurements of blood pressure, temperature, heart rate etc. Observe residents under treatment to identify progress, side-effects of medications etc. Monitor residents’ condition including fluid intake and output and compose patient charts. Administer injections, prescribed medications, enemas etc., clean and dress wounds. Collect samples for lab testing as assigned. Provide emotional and psychological support when needed. Communicate with residents’ family or friends to provide advice, comfort, and release instructions. The Pavilion at Queens for Rehabilitation & Nursing is a 302-bed skilled nursing and rehabilitation facility located in Flushing, Queens. Job responsibilities include but are not limited to. The ideal candidate will oversee the total care and safety of all residents and employees for assigned unit during shift. Supervises and assists all nursing personnel in their work performance when necessary. Document patient histories, administers medication and care as needed, coordinate necessary appointments. Monitor residents to ensure that the plans are being followed and that the resident’s and family needs are being met. Plan, coordinate, and direct all nursing staff on all shifts on the assigned units. Minimum Qualifications include: Currently licensed as Registered Nurse in New York State. Minimum 2 years supervisory experience in a long-term health care setting. Strong leadership and management skills. Has knowledge of Federal and State health regulations. Perform client triage, assessments, and monitors for symptoms of acute withdrawal and/or symptoms of intoxication and the client’s response to intervention and treatment. Closely monitors clients during administration of medications. Ensures medication schedule is timely, and adheres to unit policy and protocol. Completes required documentation in a timely and accurate manner. Able to deliver shift to shift report and communication that supports consistent client follow up. Complete nursing treatment plan that comprehensively endorses client needs and available resources. Participates in quality improvement projects. Take direction from immediate supervisor, and operate in a shift leadership role; all as part of a multi-disciplinary team/ program. QUALIFICATIONS: Currently licensed by the State of New York. Long Term Care, Assisted Living, or Hospital experience required. Candidates must be a current NYS Registered Nurse (RN). Job responsibilities include Completing accurate assessments, MDS & care plans as assigned. Monitors MDS and care planning documentation for all residents; ensures documentation is present in the medical record to support MDScoding. Initiating care plans and supporting activities as assigned. Maintaining & updating all care plans and assessments as required. Monitoring & auditing clinical records, ensuring accuracy & timeliness.
