The Avenue Care Center is looking for caring and compassionate Nurses (LPN or RN) who strive to provide superior clinical care to our residents. In joining our team of healthcare professionals at The Avenue Care Center, you will be expected to perform in a fast-paced environment and provide personalized care. The LPN/RN Charge Nurse is responsible for providing nursing care services to the residents of The Avenue. He/she will administer and record medication and treatments, instruct nursing assistants on proper resident care, and coordinate care with other medical professionals for residents as necessary. Additional responsibilities include: Ensuring resident medical needs are being addressed Providing physical support to assist patients to perform daily living activities Ensuring the patient receives appropriate nutritional needs based on care plans Recording vital signs, such as temperature, blood pressure, pulse, or respiration rate Gathering information from caregivers, nurses, or physicians about patient condition, treatment plans, or appropriate activities Observing or examining patients to detect symptoms that may require medical attention Documenting or otherwise reporting observations of patient behavior, complaints, or physical symptoms to nurses #INDHC Certification as an LPN or RN is a requirement for this opportunity. As the Night Shift Nurse Supervisor, you’ll play a vital role in ensuring our residents receive high-quality care and support throughout the night. You’ll be the primary clinical leader during overnight hours, working closely with administrative and medical staff to maintain safety, comfort, and continuity of care. Collaborates with other departments as needed (e.g., dietary, pharmacy, therapy) to support resident care. Supervises nursing personnel across all units during the night shift. Supports staff education, orientation, and training as needed. Reviews and updates nursing care plans to reflect current resident needs. Completes assigned duties and projects as directed by the DON or ADON. Provides a detailed written report to the DON summarizing overnight activities and incidents. Assists in implementing directives from the DON and supports continuity of care. Conducts periodic audits of resident care and compiles reports for review. Manages nursing-related issues and emergencies during night hours. Ensures follow-up on pharmacist recommendations and medication protocols. Provides direct care when necessary due to staffing shortages or emergencies.
