Responds to resident requests by assisting to bathroom, walking, medication requests, etc. Provides direct patient care including repositioning, hygiene, assessing skin needs, etc. Receives information from prior shift; reports information to following shift. Verifies narcotics counts at shift beginning and ending. Requests medications from pharmacy; enters new medication orders; transcribes orders; faxes orders to pharmacy; pulls medication to return. Reports to oncoming shifts. Other duties as assigned. Performs and assists with mobility, transferring, ambulation, positioning and range of motion per the care plan. Assists in orienting residents/tenants upon admission and throughout stay as needed. Documents accurately all care given, including dietary intake and output, temperature, weight, blood pressure pulse and respiration, using approved abbreviations and descriptive terms. Reports to nurse on units for written and verbal assignments which include daily use of care plans for assigned residents/tenants before initiating care. Participates in the planning of resident care through shift reports and resident care staffing meetings when assigned. Informs verbally and/or submit in writing any observations and changes in a resident for the licensed nurse to evaluate including dietary intake. Serves residents/tenants their meals; motivates and assists them with intake of nourishment and fluids. Answer all call signals and requests of residents/tenants immediately. Encourages/assist residents/tenants in keeping their rooms clean and orderly; assists with making beds. Maintains nursing unit in a clean and orderly manner. Keeps all residents/tenants’ personal belongings and equipment clean. Escorts residents/tenants to appointments and/or therapy departments as scheduled. Participates in unit activity programs planned for residents/tenants as needed. Attends and participates in unit nursing staff meetings and mandatory in-services. Performs a total inventory of all resident’s possessions on admission, discharge or transfer. Ensures all residents/tenants’ possessions are properly labeled. Responds to resident emergency including bleeding, falls, seizures and performance of Heimlich maneuver. Assists with post-mortem care as assigned. Transports dietary carts and linen carts.
