Position: MDS Coordinator License Requirement: Registered Nurse (RN) Schedule: Full-time Responsibilities: Oversee and manage the full RAI/MDS process in accordance with federal and state guidelines. Accurately assess residents and complete MDS documentation within required timeframes. Collaborate with interdisciplinary team members to ensure comprehensive resident care plans. Monitor quality measures and support compliance initiatives. Educate staff on documentation standards and care plan expectations. Qualifications: Active New Hampshire RN license required. Previous MDS or LTC assessment experience preferred. Strong understanding of the RAI/MDS process, PDPM, and care planning. Excellent communication, organization, and teamwork skills.
