OVERVIEW: Provide quality nursing care to residents in a long-term care environment which promotes their rights, dignity, freedom of choice, and their individuality under the supervision of a Registered Nurse. Interact with residents in a manner that displays warmth and promotes a caring environment · Fully understand all aspects of residents’ rights, including the right to be free of restraints and free of abuse · Responsible for promptly reporting to the charge nurse or administrative staff incidents or evidence of resident abuse or violation of residents’ rights · Assist in maintaining a safe, neat, and clean environment; report environmental deficiencies to the charge nurse such as lighting or equipment problems · Protect the personal belongings of each resident including eyeglasses, dentures, hearing aids, furnishings, jewelry, clothing, memorabilia, etc. Promptly report missing items according to established policy and participate in efforts to locate missing items · Observe residents for changes in condition or behavior and promptly report these changes to appropriate licensed nursing personnel · Provide care that maintains each resident’s skin integrity to prevent pressure ulcers, skin tears, and other damage by changing incontinent residents, turning, repositioning immobile residents, and applying moisturizers to fragile skin, etc. · Answer residents’ call bells promptly and courteously · Perform various tasks assigned by the charge nurse including vital signs, residents’ weights, applying creams/ointments, collecting specimens, etc. · Assist with orienting residents and their families to the nursing home upon admission and to the unit when transfers occur · Assist and escort residents for appointments such as beauty shop, activities, church services, etc. · Complete certified nursing assistant records while using proper coding of activities of Daily Living (ADLs) documenting care provided or other information in keeping with department policies · Follow prescribed safety and infection control procedures including thorough hand washing, use of disposable gloves where indicated, and proper disposal of soiled materials · Support an atmosphere which promotes a homelike environment · Assist in the Quality Assurance and Performance Improvement (QAPI) process QUALIFICATIONS: EDUCATION / EXPERIENCE: · A high school diploma or equivalent; successful completion of a state-approved Nursing Assistant Certification course or other education which meets state certification requirements · Must hold current Nursing Assistant Certifications in the state in which they are practicing · Must be knowledgeable of resident rights and ensures an atmosphere which allows for the privacy, dignity and well-being of all residents in a safe, secure environment. · Current CPR (American Heart Association) SUPERVISORY RESPONSIBILITIES: · This position has no supervisory responsibilities. OVERVIEW: Responsible for the overall operation and optimal quality of care for the residents on the assigned unit. Follows all health, sanitary and infection control policies and maintains established standards of practice set forth by the facility's administration and the Nursing Policies and Procedures. As an LPN you will be responsible for providing residents assistance with ADLs and in the administration of their medications while overseeing staff. Adhering to the instructions of Nurse Management, you will perform your duties in accordance with all company policies and procedures, while providing resident care services with kindness and compassion to the residents. RESPONSIBILITIES: · Holds daily shift report with nursing assistants assigned to the unit prior to the start of the shift. Reviews written daily assignments for each resident on assigned unit. Assures assignments are completed before the end of the shift · Makes rounds on assigned residents at least once every two hours · Administers medication and treatments according to the physician's orders. Assumes responsibility for ordering medications · Collaborates with Care Plan Coordinator to develop and maintain nursing care plans and update these plans on a monthly basis or more often if the resident's condition changes · Assists the attending physician with rounds and special procedures as needed · Transcribes, documents and implements physician's orders · Contacts the resident's physician and family to notify them of a change in the resident's condition or accident/incident; also notifies the supervisor covering · Assumes independent responsibility for yearly written evaluations of nursing assistants and documents accurately their strengths, weaknesses and the steps taken to improve their performance. OVERVIEW: Under the direction of the Director of Nursing, supervises the nursing personnel and the day-to-day nursing activities of the facility during an assigned tour of duty. Such supervision must be in accordance with accepted professional standards and current federal, state and local regulations to ensure the highest degree of quality care is always maintained. RESPONSIBILITIES: · Monitor the daily delivery of nursing care and nursing staff performance as they deliver nursing care to the patients/residents in accordance with established policies and procedures · Plan and conduct shift staff meetings on a regular basis to assist in identifying and correcting problem areas and to assist in the improvement of services · Identify problems or potential problems in the delivery of nursing care to residents and implement corrective action immediately · Make written and oral reports/recommendations to the Director of Nursing as necessary · Assist in developing methods for coordinating nursing services with other resident services to ensure the continuity of the resident's total regimen of care · Begin investigation of all accidents, including staff interviews, data gathering and report writing · Interpret the nursing department's policies and procedures to personnel, resident, visitors and government agencies as required · Ensure staff always maintain confidentiality of resident information · Develop work assignments and/or assist in completing and performing such tasks · Delegate to each Charge Nurse the administrative authority, responsibility and accountability necessary to perform his/her assigned duties · Make daily rounds of assigned units at a minimum at the beginning and end of the shift, depending upon the activity of the units · Follow-up on complaints and grievances filed by resident, families or staff. OVERVIEW: Assist the Director of Nursing and Assistant Director of Nursing in planning, developing, organizing, implementing, evaluating, and directing the day-to-day functions of the Nursing Department. Maintains quality clinical care for residents/patients in accordance with current rules, regulations, and guidelines that govern the Facility. RESPONSIBILITIES: · Supervises the clinical care planning and delivery processes to ensure the coordination of quality patient care.
